- NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN.
- ELIGIBILITY: The Fuel Up to Play 60 Contest (the "Contest") is open only to legal residents of the 50 United States and the District of Columbia who are enrolled in Grades K through 12 (minimum age of 6, maximum age of 17) as of 9/1/09. The children of employees of Dairy Management Inc., the NFL Entities (as defined below), any of their respective parent companies, subsidiaries, affiliates, member teams, advertising and promotion agencies and agents are NOT eligible to enter the Contest. Students do not need to be enrolled in a specific or participating school to be eligible.
- TIMING: Contest starts on 10/15/09 at 12:00 am CST and ends on 3/15/10 at 11:59 pm CST.
- HOW TO ENTER: Students: To compete for awesome prizes for you and (if applicable) your school in the Contest, sign up by registering at www.FUELUPTOPLAY60.com. (the “Site”) You must provide us with your parent’s valid e-mail address at registration or add it prior to 11:59 pm CST on December 15, 2009 to be eligible to win the individual midpoint prize and 11:59 pm on March 15, 2010 to be eligible to win the individual grand prize. Schools: Have your principal or administrator enroll for the program at www.schoolwellnesskit.org. Sorry, but you must enter and play on line to participate in the Contest. If you cannot get to a computer everyday to participate, print off the paper tracker in the Track section, fill out the paper tracker everyday and enter your paper tracker data online once a week. Make sure you do it weekly because you can only track back one week. If you do not have internet access but would like to learn about staying healthy through proper eating habits and exercise please call (800) 752-4337.
- HOW TO PLAY: You can earn points for you and your school by engaging in activities on the site and having friends enter the Contest too.
Here is how you can earn points for you and your school:
Students
- Take the pledge during registration: Earn 100 points one time for taking the pledge.
- Eating Healthy: Earn points for eating the recommended servings of healthy foods every day and recording it in the Track section on the Site. Earn 10 points each day for up to 3 dairy servings, 3 whole grains servings, 3 fruit servings and 5 vegetable servings, up to a maximum of 140 points per day (see www.mypyramid.gov for recommended serving information).
- Stay Active. Earn points for getting the recommended amount of physical activity everyday and recording it in the Track section on the site. Earn 10 points for every 5 minutes of exercise Tracked, for up to 60 minutes/120 points per day.
- Take Action. Earn points by taking action, such as encouraging friends to check out the Site or taking other specified actions described in the Tell us What you Did Today banner in the Track section of the Site and recording the activity in the Track section of the site. Earn up to 100 points for each action, up to 1 time per session and 2 times total per action for the program.
- Score on “Pop” Quizzes and Challenge Activities. On Tuesdays and Thursdays, users will be asked a healthy/active lifestyle question at the completion of tracking their activity for the day. Each week users will be asked a challenge question in the My Stuff section regarding the Fuel Up to Play 60 Program and the Playbook. You can earn 50 points by answering each quiz or challenge question correctly.
- Refer friends on the Fuel Up to Play 60 Web site and earn even more points. Throughout the Site, users can click on tell a friend, provide a friend’s e-mail address and that friend will receive an e-mail encouraging them to sign up for the program. The referrer will score points when the recipient clicks on the link in the e-mail to register. Each e-mail address can only be referred once by each user. Earn 50 points for each unique valid e-mail address referred. Referring e-mails must be sent to friends only. Complaints of excessive or spam e-mails will result in disqualification.
School: Schools will get judged based on the participating rate of students, so the more classmates register and earn points the better chance your school will have to win.
Here are the prizes you and your school can win.
- PRIZES/AWARDING: Student: National Grand Prize (1): The student with the most points will win the opportunity to appear in a Fuel Up to Play 60 promotion with an NFL Player and opportunity to train with and get nutrition tips from an NFL player. NFL Player appearances are subject to availability. Parent or guardian must accompany winner during time with player. Prize includes winner and parent or guardian travel expenses, if required (coach airfare on regularly scheduled airlines, hotel room and local transportation) up to a maximum of $2,500 in order to appear in promotion and spend time with player, provided all travel arrangements are made by Sponsor. No other expenses are included in prize. (APR $2,500). Mid-Point Prize (50): The student with the highest point total in each State and the District of Columbia as of 12:00 AM CST on December 15, 2009 will receive an Apple iPod touch™, a Fuel Up to Play 60 cover and a $50 gift certificate from NFLShop.com (APR $355). Odds of winning depend on the number of eligible entries received. (Total APR of skill contest prizes $18,750) If you are declared a potential winner, an e-mail will be sent to your parent’s e-mail address or mailing address provided. Your parent will have 7 days to respond to the e-mail to confirm they will return any requested documents in order to obtain the prize. If your parent does not respond by 12 AM on the 7th day, your prize will be forfeited and the runner up will be contacted. In the event of a tie, the tied students with the highest score on physical activity will be compared and the highest score will win, if students scores are still tied, the tied student with the highest score in healthy eating will win, if the scores are still tied the tied students with the highest score in Tell Us What you Did today will win, if the scores are still tied the students with the highest quiz score will win, if the score is still tied, the students with the highest challenge score will win and if the score is still tied, the students with the highest tell a friend score will win. In the event students are still tied, the remaining tied students will be asked to submit an essay (400 words or less) explaining: A) How did you make your school or community a better place by participating in the Fuel Up to Play 60 Program; and B) How did you motivate others to Participate in the Fuel Up to Play 60 program. Essays will be judged according to the following criteria; Compatibility with theme 50%; Creativity 25%, and Style and Grammar 25%. Point level prizes: By achieving specific levels of points, students will receive additional unlocked content on the Site as follows: 100 points-Fast Sack Attack NFL Game; 5,000 points-Leaping Lineman NFL Game; 10,000 points-Quarterback Smartathon NFL Game; 20,000 points-Scoreboard Freakout NFL Game; 30,000 points -Match in This NFL Game; 40,000 points NFL Hall Pass; and 50,000 points NFL Puzzle Rush. School: Grand Prize: The school with the highest fuel meter will be featured in a Fuel Up to Play 60 promotion with an NFL player and will receive a Fuel Up to Play 60--themed “healthy school makeover,” including a gym makeover (valued at $20,000), offering new sports gear, such as jump ropes, yoga mats, footballs and more, as well as a cafeteria makeover (valued at $20,000). The school with the highest fuel meter in each participating state and the District of Columbia will receive a prize package to help them showcase their achievements (valued at $1,000). Prize package will be discussed between Sponsor or local dairy council representative and the school to determine exact components. In the event of a school vs. school tie, the tied schools with the highest score on physical activity will be compared and the highest score will win, if schools scores are still tied, the tied school with the highest score in healthy eating will win, if the scores are still tied the tied schools with the highest score in Tell Us What you Did today will win, if the scores are still tied the school with the highest quiz score will win, if the score is still tied, the school with the highest challenge score will win and if the score is still tied, the school with the highest tell a friend score will win. In the event students are still tied, the remaining tied students will be asked to submit an essay (400 words or less) explaining: A) How did you make your school or community a better place by participating in the Fuel Up to Play 60 Program; and B) How did you motivate others to Participate in the Fuel Up to Play 60 program. - PRIZES, RESTRICTIONS: Limit one prize per person and per school. In the state with the national school winner, the second place school will be declared the state winner. Prizes are non-transferable and must be accepted as awarded, except Sponsor may substitute a prize of equal or greater value. In the event an NFL player cannot fulfill the prize by appearing in the promotion or attending the day to hang out with an NFL player for any reason whatsoever, including but not limited to scheduling conflicts, cancellations, postponements, an event of force majeure, or for any other reason, then Sponsor may substitute an NFL merchandise package of $1,000 for the Student Grand Prize of a day to hang out with an NFL player and the Individual and School Grand prize of a starring role in a Fuel Up to Play 60 promotion. Parent or guardian may be required to be present during NFL player visit. NFLShop.com gift certificates subject to all gift certificates terms and conditions. Taxes, if any, are the responsibility of the winner.
- JUDGING: All eligible entries received will be judged by a panel of judges (the "Judges") drawn from employees of the Sponsor. Judges’ decisions are final and may not be appealed. Potential winners will contacted through an e-mail to the parent e-mail address provided at registration. If a parent cannot be contacted or does not return fully completed documentation within the time specified, another winner will be chosen. Parents of potential winners will be required to complete, execute and return an Affidavit, Waiver, Release and Indemnity Agreement including where lawful, a Publicity Release (plus at the discretion of the Sponsor, any additional documents) and a W-9 form (where applicable). Winners will be required to sign Affidavits of Eligibility confirming that they have committed no fraud in connection with the Contest and Publicity (where applicable) and Liability Releases. Failure to comply with the requirements of this paragraph (and/or the return of a prize or parent notification and/or prize as non-deliverable) may result in disqualification and in the forfeiture of any right to a prize in which case, the prize may be awarded to the eligible entrant ranked next in line for the relevant prize award.
- RELEASE: As a result of the submission of participating in the Contest, and/or as a result of agreeing to the acceptance of a prize, all entrants agree: a) To defend, release and hold harmless the Sponsor,the NFL Entities (as defined below),its respective parents, subsidiaries, affiliates, divisions, advertising and promotion agencies, and the foregoing companies’ officers, directors, shareholders, employees, representatives, and agents (collectively "Releasees") from and against any actions, claims and/or liability for injury, loss or damage of any kind (including any for the violation or infringement of any proprietary or personal right of any individual or entity) resulting in whole or in part, directly or indirectly, from participation in this Contest, and/or the use, acceptance, or possession of a Contest prize, and/or participation in, or attendance at a Contest prize activity; b) To consent to and give the Sponsor the right in perpetuity to use demographic data collected, a Parent’s and/or entrant’s city of residence, picture, portrait, likeness, voice, video, statements, quotes, prize information, submitted entry and submitted essay (in whole or in part, as may be adapted, edited, cropped or modified, as solely determined by Sponsor) for advertising, trade, publicity and promotional purposes in any media now known or hereafter discovered, worldwide, and on the World Wide Web, without review, notification or approval, all without additional consideration, unless prohibited by law; c) To release the Releasees from any liability in connection with entries that are ineligible as a result of being lost, late, illegible, damaged, incomplete, stolen, postage due, or misdirected; d) To release the Releasees from any liability in connection with any typographical or other human or technical errors in the offer or administration of this Contest, including but not limited to errors in advertising, these official rules, the selection and announcement of a winner(s), or the distribution/awarding of a prize(s). All entries are the property of DMI and will not be returned.
- CONDITIONS: Sponsor reserves the right to modify these rules for clarification purposes without materially affecting the terms and conditions of the Contest. In the event that the Contest is infected by a computer virus/worm, or is not capable of running or being executed as planned (as a result of but not limited to an error, omission, defect, delay, misdirection, tampering, unauthorized intervention, fraud, action of entrants, or technical failure) or any other cause which in the sole opinion of Sponsor corrupts or affects the administration, security, fairness, integrity, or proper conduct of the Contest, Sponsor reserves the right in their sole discretion to disqualify any suspect entry or entrant and to cancel, terminate, modify or suspend this Contest. In the event of such cancellation, termination, modification or suspension, notice thereof will be posted at the Site and winner(s) will be determined from among all eligible non-suspect and/or non-disqualified entries using the judging criteria mentioned above. In such event, Releasees shall have no liability to any participant who is disqualified due to such an action. Failure to enforce any term of these official rules shall not constitute a waiver of that provision. Submitted entries shall be deemed to be works-made-for-hire owned by Sponsor under the copyright laws of the United States, and submission of an entry by an entrant shall constitute and be deemed to be the corresponding Parent’s irrevocable consent (on behalf of themselves and the entrant) to transfer and assign all rights, title and interest in a submitted entry and submitted essay to Sponsor, including but not limited to the copyright, and renewal rights worldwide, and to execute all documents and take all actions deemed reasonably necessary by Sponsor to effect, record and perfect any requested assignment. Submitted entries will not be acknowledged or returned.
- LAWS: Subject to all federal, state, and local laws and regulations. Void where prohibited.
- WINNERS’ LIST: The names of winners will be listed on the Site on or about May 15, 2010 for grand prize winners.
- PRIVACY: Sponsor is committed to protecting the privacy of children. This Contest is being operated in compliance with the Children’s Online Privacy Protection Act (“COPPA”), and no personal information regarding entrants is maintained except in compliance with COPPA.
