FOR PARENTS

Hello Moms, Dads, Guardians and Caregivers!

This privacy policy explains our practice for information collection and use for visitors to the Fuel Up to Play 60 website. This privacy policy applies to information submitted by visitors, including kids, who use www.FuelUpToPlay60.com for participation in the Fuel Up to Play 60 online program.

CHILDREN'S PRIVACY

When a student registers we ask them to provide a username and their date of birth. We do not store date of birth for users under the age of 13.

If your child is under age 13, we do not request any personal information (as defined below) from your child until we receive permission from you to collect their Personal Information.

The parent permission form (English or Spanish) must be completed, signed and returned to us using the instructions on the form.  The permission form requests the following information about your child: first and last name, Fuel Up to Play 60 username, date of birth, gender, school name and school zip code.  We also request that parents provide their first and last name and email address.

The process to verify parental permission may take up to 48 hours.

Once parent permission has been provided, we will request that your child provide us with his or her first and last name, email address, school ZIP code and school name. The child may also choose to provide the following optional information: gender, a parent’s cell phone number, and a mailing address.

Providing a parent’s cell phone number is optional, and may be removed from a child’s account at any time. We collect a cell phone number to contact you if there is a problem with the under 13 parental permission form, to send program notifications, to provide each student with an individualized experience on the site, and as an alternative means of communicating with students for special promotions and contests. We collect a mailing address to contact you if your child has applied to attend the Student Ambassador Summit or another Fuel Up to Play 60 event, and as an alternative means of communicating with students for special promotions and contests.

The student’s online account can be linked to his or her school location. This is the only location data is stored in the database. This link between student and school allows students to earn Points and have the Points count toward their school’s total score. It allows students to view school specific messaging on their Dashboards, and allows participation in Local Dairy Council or Program Advisor (school) competitions and contests. Without this information, the student is ineligible to participate in State, Regional, and National contests

If parental permission is not received, the student can still earn Points. Students under 13 will not receive program communications or be eligible to participate in contests until parental permission is received.

Every year, there is a Fuel Up to Play 60 Student Ambassador Summit. If your child is qualified to attend the event, they will have the opportunity to complete an application to attend. On that application, we will ask for your child’s mailing address, which will include street address, city, state and zip code and a parent’s phone number. The mailing address and parent’s phone number will be used to contact you and your child about the event.

We will only use Personal Information for the purpose for which it is collected. We do not share Personal Information with third parties except on a confidential and secure basis to those parties that help us administer our website or related activities.

The School Stats includes a link to a list of all adults enrolled at the school. The list is provided online via a pop-up screen when you click on the ‘view list’ link and provides the first and last name of the registered adult, as well as whether or not they are a Program Advisor.  This all happens post-log in.

When a student 13 or older registers we ask them to provide a username and their date of birth.

We request an email address because it is the default method of communication with students. It also helps to identify the student, validate login credentials, and recover a username. If a student 13 or over does not have an email address, it is recommended that he or she set up a free email account through a free email service provider.

Students may choose to provide a cell phone number which we ask be the parent’s number. We collect a cell phone number, to send program notifications, to provide each student with an individualized experience on the site, and as an alternative means of communicating with students for special promotions and contests. Providing a cell phone number is optional and may be removed at any time.

Students may choose to provide a mailing address. We collect a mailing address to contact you if your child has applied to attend the Student Ambassador Summit or another Fuel Up to Play 60 event, and as an alternative means of communicating with students for special promotions and contests.

In all cases in which we collect Personal Information, we specifically describe what information is collected in order to provide you with the activity, service, or feature you have requested. At no time does FuelUpToPlay60.com collect or store any other location data besides the information that links a student to his or her school.

The Fuel Up to Play 60 website collects Personal Information when you contact us, when you register, when parental notice or consent is required, when you use any and all related activities, services, or features, when you participate in a contest, and when you refer your friends to the website.
