Collection of Personally Identifiable Information

The Fuel Up to Play 60 website may disclose the full names of adults enrolled in the Fuel Up to Play 60 website (such as Program Advisors, enrolled educators, and other enrolled adults) to other adults enrolled in the Fuel Up to Play 60 website.

If your child wants to register on the Fuel Up to Play 60 website, we will request that your child provide us with his or her first and last name, email address, parent’s cell phone number (optional), parent’s email address, gender, grade, and ZIP code. If your child is under age 13, we will only request this information from your child after we receive permission from you to collect Personally Identifiable Information (as defined below) from your child. Providing a parent’s cellphone number is optional, and may be removed from a child’s account at any time. We collect a cell phone number to contact you if there is a problem with the under 13 parental permission form, to send program notifications, to provide each student with an individualized experience on the site, and as an alternative means of communicating with students for special promotions and contests. Because the registration application asks for some Personally Identifiable Information (as defined below), students under the age of 13 are required to have parent permission. The parent permission form (English or Spanish) must be completed, signed and returned to us using the instructions on the form.

We may also keep records of non-personally identifiable or aggregate data, such as the number of hits a Web page receives, or the IP address of the computers accessing the site. We do not link this data with any Personally Identifiable Information (as defined below). Third parties that help us administer the website may have confidential access to this aggregate data.

Other Privacy Information

We will only use Personally Identifiable Information (as defined below) for the purpose for which it is collected. We do not share Personally Identifiable Information (as defined below) with third parties except on a confidential and secure basis to those parties that help us administer our website or related activities. The information provided to www.FuelUpToPlay60.com will not be rented or sold to any third party.

We may disclose you or your child's Personally Identifiable Information (as defined below) to third parties in order to comply with applicable law, to cooperate with law enforcement agencies as required by a valid legal process (such as a court order or a subpoena), to protect our rights, or to protect the rights of other users of this website.

Fuel Up to Play 60 has received TRUSTe’s Children’s Privacy Seal signifying that this privacy policy and our practices have been reviewed for compliance with the TRUSTe program viewable on the validation page by clocking the TRUSTe seal. The TRUSTe certification covers information collected through our site www.fueluptoplay60.com. If you have questions or complaints regarding our privacy policy or practices, please contact us. If you have an unresolved privacy or data use concern that we have not addressed satisfactorily, please contact our U.S.-based third party dispute resolution provider (free of charge) at https://feedback-form.truste.com/watchdog/request.

This Privacy Policy also describes the choices available to you regarding the use of the Personally Identifiable Information (as defined below) that you and/or your child provide to the Fuel Up to Play 60 website, as well as the actions that can be taken to access this information and request that the information that you and/or your child provides be corrected or deleted.

Children's Online Privacy and Protection Act Compliance

The Children's Online Privacy and Protection Act of 1998 requires websites that collect and store data from users under thirteen years old to inform parents about data storage policies and obtain parental permissions. Once given, parents may revoke those permissions at any time informing us of their decision.

Personally Identifiable Information

"Personally Identifiable Information" means information that can identify a specific individual, including those under 13 years of age, such as your name, address, phone number, email address, or other contact information, whether at work or at home, and in some cases a username. Personally Identifiable Information that we collect includes first and last name, student and parent email addresses, and cell phone number.

We also collect, gender, grade, state, zip code, school, username and password that, in and of itself, is not personally identifiable.

Date of birth is required to determine the student’s age. Those over 13 years of age may continue with the registration process. Those under 13 years of age may create a username and password and are able to earn Points on their Dashboards prior to receiving parental consent. Students must obtain parental consent (as defined below) to continue the registration process and will not be asked for any Personally Identifiable Information until consent has been obtained. They are not eligible to enter or win sweepstakes and Fuel Up to Play 60 will not contact this student in any way.

Students under the age of 13 will be prompted to obtain parental permission when they earn 30,000 Points. When the student earns 30,000 Points, a message will appear on the student’s Dashboard that prompts the student to download the parent permission form and have a parent complete and return the form to Fuel Up to Play 60.

A paper Parental Permission Form in English or Spanish will be available for download on the student Dashboard once the student reaches 30,000 Points. The paper form can be downloaded, printed, signed and sent back to Fuel Up to Play 60. Fuel Up to Play 60 will confirm approval of the form. The form may be accessed here. (Also available in Spanish here.)

The process to verify parental permission may take up to 48 hours. Once parental permission is received, the student will be able to access the My Account page on his or her Dashboard and update his or her profile by adding, his or her first and last name, email address, grade, and ZIP code. The student’s online account can be linked to his or her school location. This is the only location data stored in the database. This link between student and school allows students to earn Points and have the Points count toward their school’s total score. It allows students to view school specific messaging on their Dashboards, and allows participation in Local Dairy Council or Program Advisor (school) competitions and contests. Without this information, the student is ineligible to participate in State, Regional, and National contests

First and last name is requested to help validate the student’s profile credentials when the student requests to retrieve his or her username. First and last name are also used to accurately enter students into program contests and accurately award them for contests and activities.

We request an email address because it is the default method of communication with students. It also helps to identify the student, validate login credentials, and recover a username. If a student 13 or over does not have an email address, it is recommended that he or she set up a free email account through a free email service provider. Parental permission allows Fuel Up to Play 60 to communicate with students under 13 years of age. We ask that the parent’s email be provided for students under 13 years old.

Students may choose to provide a cell phone number which we ask be the parent’s number. We collect a cell phone number to contact you if there is a problem with the under 13 parental permission form, to send program notifications, to provide each student with an individualized experience on the site, and as an alternative means of communicating with students for special promotions and contests. Providing a cell phone number is optional and may be removed at any time.

If parental permission is not received, the student can still earn Points. Students under 13 will not receive program communications or be eligible to participate in contests until parental permission is received.

In all cases in which we collect Personally Identifiable Information, we specifically describe what information is collected in order to provide you with the activity, service, or feature you have requested. At no time does FuelUpToPlay60.com collect or store any other location data besides the information that links a student to his or her school.

The Fuel Up to Play 60 website collects Personally Identifiable Information when you contact us, when you register, when parental notice or consent is required, when you use any and all related activities, services, or features, when you participate in a contest, and when you refer your friends to the website.

We collect and store Personally Identifiable Information from all users for the purposes of providing an individualized user experience and validating access rights to the desired account.

Registration for The Fuel Up to Play 60 Website

If your child wants to register on the Fuel Up to Play 60 website, we will request that your child provide us with his or her first and last name, email address, your email address, your cell phone number (optional), gender, grade, school, and ZIP code. If your child is under age 13, we will only request this information from your child after we receive permission from you to collect Personally Identifiable Information from your child. A completed and signed parent permission form is required to complete the registration process for those students under 13 years of age. This form can be downloaded (English or Spanish) and returned via fax, mail or email.

You are required to register in order to use select features of the Fuel Up to Play 60 website such as being able to access the student Dashboard, entering sweepstakes, or updating profile information. However, a child’s participation or access to an activity cannot be conditioned on him or her providing more Personally Identifiable Information than is reasonably necessary for that activity.

Parents have the right to refuse further collection and use of their child’s Personally Identifiable Information, and to request the deletion of such Personally Identifiable Information. Please see the “Access to Your Information” section for instructions on how.

If you participate, you will be required to register with the Fuel Up to Play 60 website, which will require you to provide certain Personally Identifiable Information, including first and last name, email address, parent’s email address, parent’s cell phone number, age, gender, state and ZIP code. Participation is voluntary and you therefore have a choice whether or not to register and, if parental permission is granted, provide your Personally Identifiable Information.
