The Kickoff (“Contest”) is in no way sponsored, endorsed, or administered by, or associated with YouTube. By entering the Contest, you are agreeing to the rules set forth below and releasing YouTube, from any liability.

1. Eligibility: The Kickoff Contest is open to FuelUpToPlay60.com website (“The Site”) account holders 18 years of age and older. Limit one (1) entry per person, per Entry Period. Employees of Dairy Management Inc., National Dairy Council, NFL Entities (as defined below), NFL Players Association, NFL PLAYERS Inc., their respective parents, affiliates, subsidiaries, advertising or promotion agencies or other individuals engaged in the development, production, or distribution of promotion materials and persons who are immediate family or who reside in the same household as persons in the preceding categories are not eligible. 2. How to Enter:

First Prize sweepstakes: Beginning at 12:00:01 am Eastern Time (“ET”) on 8/12/2019 and ending at 11:59:59 pm ET on 11/15/2019 (the “Entry Period”), eligible entrants will automatically receive one (1) entry into the First Prize sweepstakes when they log in to their account on FuelUpToPlay60.com (the “Site”) and report a Kickoff under the Kickoff Step of the Touchdown Steps on the educator account Dashboard. Limit one (1) entry per eligible entrant. At the conclusion of the Entry Period, fifty (50) First Prize winners from the pool of eligible First Prize sweepstakes entrants will be randomly selected to win $200 gift card to the online Fuel Up to Play 60 Store, www.bdasites.com/FuelUpToPlay60/Main/Default. Sponsor reserves the right in its sole discretion, to disqualify any individual who tampers with the entry process. Entries may be subject to verification of eligibility. Sponsor is not responsible for lost, late, misdirected, damaged or incomplete entries, or for transactions that are processed late, incorrectly computed, or are lost because of computer error or malfunctions, all of which are void.

Grand Prize Say Cheese Photo Entry: During the Entry Period, eligible entrants will receive one (1) entry for the Grand Prize when they create a Say Cheese Photo Entry and either publicly post it on Facebook, Twitter or Instagram, or, email the photo to studentambassador@fueluptoplay60.com, meeting all of the following Say Cheese Photo Entry Requirements during the Entry Period (“Post”):

Photo Entry Requirements:

  • Required hashtags must be used:  #FuelGreatness and #FUTP60Contest
  • Must be posted to a Facebook, Twitter OR Instagram account – OR – emailed to studentambassador@fueluptoplay60.com with required information (name, school name, location)
  • School nutrition staff must be pictured
  • FUTP 60 Team Spirit must be represented
  • Cheese must be incorporated
  • Entrants must obtain any required permissions from featured students/educators to publicly use their image
  • Cannot feature more than three pieces of PII (Personally Identifiable Information) for students; Educators uploading the video must not disclose the full name and location of a student.
  • Must comply with all applicable Facebook, Twitter or Instagram Terms of Service, Privacy Policies, and Community Guidelines available on the corresponding websites.
  • Must comply with all Kickoff Official Rules contained herein.

Limit one (1) entry per eligible participant. At the conclusion of the Entry Period, contest judges will select one (1) Grand Prize winner to receive an in-school cheese party with an NFL player and other special guests. Sponsor reserves the right in its sole discretion, to disqualify any individual who tampers with the entry process. Entries may be subject to verification of eligibility prior to judging. Sponsor is not responsible for lost, late, misdirected, damaged or incomplete entries, or for transactions that are processed late, incorrectly computed, or are lost because of computer error or malfunctions, all of which are void.

3. Grand Prize Say Cheese Photo Entry Judging Criteria:  Within a period of 14 days after the Entry Period, the entries received during that period will be judged by a panel of judges selected by MMS Education based on the following rubric:

Video Entry Judging Rubric:

  • 40 points: Clear, compelling and creative expression of students showing how “cheesy” their team can be, and how much their school loves cheese.
  • 40 points: FUTP 60 Team Spirit
  • 20 points: Overall creativity and visual quality of photo

4. Prizes: Fifty (50) First Prize winners will each receive a $200 gift card to the online Fuel Up to Play 60 Store, www.bdasites.com/FuelUpToPlay60/Main/Default. One (1) Grand Prize winner will receive an in-school cheese party with an NFL player and other special guests. First Prize retail value: $200 per winner for a total First Prize value of $10000. Grand Prize retail value: $4,000. Total prize pool value: $14,000. Odds of winning depend on the number of valid entries received. Prizes are non-transferable. Federal, state, and local taxes, if any, are the sole responsibility of winners. Winner understands that he/she must claim a prize award on their tax return that is valued at $600.00 or greater. No cash or other substitution may be made, except by Sponsors in the event that a prize (or part thereof) cannot be awarded, in which case sponsor reserves the right in its sole discretion to substitute a prize with one of comparable or greater value. PRIZES WILL BE AWARDED “AS IS” WITH NO WARRANTY OR GUARANTEE, EITHER EXPRESSED OR IMPLIED, BY SPONSOR.

First Prize Winners will be notified via email to the email address associated with their FuelUpToPlay60.com account within two (2) weeks of the Entry Period end.

Grand prize Winners will be notified via the Facebook, Twitter or Instagram account from which they shared their winning Say Cheese Photo Entry, OR, via response to the email message from which they submitted their Say Cheese Photo Entry, and will receive their in-school cheese party with an NFL player and other special guests within 60 days of the Entry Period end.

5. Additional Terms: By accepting a prize, winner consents to the use of his/her name for advertising and promotional purposes without additional compensation, except where prohibited by law. By entering, winner agrees to release, discharge, and hold harmless Dairy Management Inc., the NFL Entities (as defined below), NFL Players Association, NFL Players Inc., their respective parents, affiliates, subsidiaries, advertising or promotion agencies or other individuals engaged in the development, production, or distribution of Contest materials (“Releasees”), from all claims or damages arising out of participation in both this offer and/or Contest and/or acceptance of any prize or offer, including any injuries or damages of any kind sustained in connection with the use, acceptance, possession or awarding of any prize. 6.  Post Requirements.  By submitting a Post you warrant and represent that:

All Posts must comply with the following guidelines. Failure to comply with these guidelines may result in disqualification of entry.

  • Posts cannot be sexually explicit or suggestive, unnecessarily violent, or derogatory of any ethnic, racial, gender, religious, professional or age group, profane or pornographic, contain nudity or any materially dangerous activity;
  • Posts cannot promote alcohol, illegal drugs, tobacco, firearms/weapons (or the use of any of the foregoing), any activities that may appear unsafe or dangerous, or any particular political agenda or message;

  • Posts cannot contain copyrighted materials owned by others (including photographs, sculptures, paintings and other works of art or images published on or in websites, television, movies or other media) without permission; Sponsor does not permit the infringement of others’ rights and any use of materials not original to the entrant (except copyrighted materials owned by Sponsor) is grounds for disqualification. Limitation of Liability: Releasees are not responsible for printing, typographical, human or other errors of any kind in any contest-related materials; for stolen, lost, late, misdirected, damaged, incomplete, illegible entries; or for transactions that are lost, misdirected, or fail to enter into the processing system, or are processed, reported, or transmitted late or incorrectly, or are lost for any reason, including computer, telephonic, paper-transfer, or human error, or electronic, computer, or telephonic malfunction, damage, error, omission, interruption, defect, delay in operation or transmission, or any combination thereof. If, for any reason, this contest is not capable of running as planned, or infection by computer viruses, bugs, tampering, unauthorized intervention, fraud, actions of participants, technical failures, or any other causes corrupt or affect the administration, security, fairness, integrity, or proper conduct of the contest, Sponsor reserves the right in its sole discretion to disqualify any entry or participant and/or to cancel, terminate, modify, or suspend the contest and award a prize from among all eligible non-suspect entries validly received prior to the action requiring such modification. By entering, all participants agree that this contest is governed by laws of the state of Illinois for the resolution of all claims and disputes (excluding any claims against or disputes involving parties other than Sponsor relating to any prize awarded in the contest). NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN.

1. Eligibility: The Super Sleep Contest (“Contest”) is open to Fuel Up to Play 60 educators and students who are ages 13+ as of October 24, 2019, who have or create an account on GENYOUth’s FuelUpToPlay60.com (“Site”), and who are U.S. citizens (“Eligible Entrants”).  Each Video Entry must be submitted by a team made up of one educator and one student, an “educator-student team.”  Each student must have provided their educator teammate an appropriate parental/legal guardian permission to enter the Contest.  Employees of Sleep Number Corporation, GENYOUth, Dairy Management Inc., National Dairy Council, NFL Entities (as defined below), NFL Players Association, NFL PLAYERS Inc., their respective parents, affiliates, subsidiaries, advertising or promotion agencies or other individuals engaged in the development, production, or distribution of promotion materials and persons who are immediate family members of or who reside in the same household as persons in the preceding categories are not eligible. Void where prohibited. 

 

2. How to Enter:

 

Sleep Number Bed Contest: During the “Entry Period,” which is [08:00 a.m. central daylight time] on October 24, 2019 to [11:59 p.m. central standard time] on December 6, 2019, eligible educator-student team entrants will be entered into the Contest when they submit a 90-second or shorter private or password-protected Dropbox, Vimeo or other video link via the Super Sleep Contest Entry Form, hosted by Survey Monkey (“Entry Form”), available at http://bit.ly/SNSuperSleep.

 

Video Entry Requirements:

  • Using the Entry Form, an eligible educator-student team must submit their Video Entry by completing the Entry Form and either: (1) providing a link to their video, if it was created using Dropbox, Vimeo, or another service provider; or (2) sending the video file by email to StudentAmbassador@FuelUpToPlay60.com.

  • Video Entry has to be accessible to the reviewing team (i.e. if password protected, a password must be provided.)

  • Must be 90 seconds or less

  • Music must be license-free

  • Must feature at least 1 fact about how sleep improves overall health and wellness and academics

  • Must feature at least 1 Super Sleep Tip for students

  • Must feature an activity that shows off what their entire school can do after everyone gets a good night’s sleep

  • Must not include any profanity or inappropriate content, including any footage of students in their bedrooms or in pajamas

  • Each educator-student team must obtain any required permissions from parents/guardians of the students to publicly use their image(s)

  • Cannot feature more than three pieces of Personally Identifiable Information (PII) for students (i.e. first name, last name, state, or school name, etc.)

  • Must comply with all Super Sleep Contest Official Rules contained herein.

 

Limit one (1) entry per eligible educator-student team. At the conclusion of the Entry Period, contest judges will select one hundred (100) educator-student team winners.  The educator of each educator-student team winner will receive a Sleep Toolkit delivered to their schools.  The student of each educator-student team winner will receive a Sleep Number Gift Certificate delivered to their home.  MANY WILL ENTER, FEW WILL WIN. Administrator reserves the right in its sole discretion, to disqualify any individual who tampers with the entry process. Video Entries may be subject to verification of eligibility prior to judging. Neither Administrator nor Sponsor are responsible for lost, late, misdirected, damaged or incomplete entries, or for transactions that are processed late, incorrectly computed, or are lost because of computer error or malfunctions, all of which are void.  By entering, entrant fully agrees to these Official Rules and Administrator’s decisions regarding the Contest, which are final and binding. Winning a prize is contingent upon fulfilling all requirements in these Official Rules.

 

3. Grand Prize Judging Criteria:  Within a period of 18 days after the Entry Period, the Video Entries that have met all Video Entry Requirements noted herein that were received during the Entry Period will be judged by a panel of judges selected by Administrator based on the following rubric:

  • 40 points: Clear, compelling and creative expression of students showing how sleep is important to them regarding health and academics

  • 40 points: Clear, authentic message about sleep

  • 20 points: Audio and visual quality (technical execution of Video Entry)

 

4. Prizes: One hundred (100) educator-student teams will win.  Each of the one hundred (100 educators of each educator-student team winner will receive a Sleep Toolkit, and each of the one hundred (100) students of each educator-student team will receive one Sleep Number Gift Certificate will be awarded to the parent or guardian of each student winner.  Sleep Toolkit value: $200.00 USD. Sleep Number Gift Certificate retail value: $1,200.00 USD. Total approximate retail value: $1,400.00 USD. Odds of winning depend on the number of valid entries received. Prizes are non-transferable. PRIZES WILL BE AWARDED “AS IS” WITH NO WARRANTY OR GUARANTEE, EITHER EXPRESSED OR IMPLIED, BY SPONSOR.

Prize Winners will be selected and notified via email no later January 6, 2020.

 

5. Additional Terms:  Neither the educator entrant nor the student entrant transfers ownership of their Video Entry by entering the Contest. However, by entering, the educator-student team grants Administrator, Sponsor, its agents, licensees, and assigns an irrevocable, perpetual (non-exclusive) right and permission to reproduce, encode, store, copy, transmit, publish, post, broadcast, display, publicly perform, adapt, modify, create derivative works of, exhibit, and otherwise use the Video Entry, name, and likeness, as-is or as-edited (with or without using the respective names of the educator or student) in any media throughout the world for any purpose, without limitation, and without additional review, compensation, or approval from entrant, except where prohibited by law. By entering, the educator-student team consents to the use of their names for advertising and promotional purposes without additional compensation, except where prohibited by law. By entering, the educator-student team  agrees to release, discharge, and hold harmless Sleep Number, Dairy Management Inc., National Dairy Council, the NFL Entities (as defined below), NFL Players Association, NFL Players Inc., their respective parents, affiliates, subsidiaries, advertising or promotion agencies or other individuals engaged in the development, production, or distribution of Contest materials (“Releasees”), from all claims or damages arising out of participation in both this offer and/or Contest and/or acceptance of any prize or offer, including any injuries or damages of any kind sustained in connection with the use, acceptance, possession or awarding of any prize, any breach of publicity rights, defamation, invasion of privacy, copyright infringement, trademark infringement, violation of law, or any other claim or damage. Administrator reserves the right to modify prize award procedures at its discretion. Subject to applicable federal, state, and local laws and regulations. Without limiting the foregoing, winners may be featured on the Administrator’s or any of the Releasees’ social media channels, and as part of their win will provide a signed waiver granting permission for appropriate parties to do so. 6.  Video Entry Requirements.  By submitting a Video Entry, all Eligible Entrants warrant and represent that:

  1. the Video Entry is an original work of the educator-student team and has not been previously published nor has it won previous awards;

  2. that the Video Entry does not violate any person’s right of privacy and that the educator-student team has received the required legal consent from all subjects in the Video Entry or the parents of all subjects in the Video Entry, as may be applicable.  Administrator reserves the right to require entrants to timely submit evidence of such legal consent and other documents, which may include a signed waiver from the subject or the subject’s parent. NOTE: Waiver requirements vary by jurisdiction, and the educator-student team is responsible for obtaining a legally enforceable waiver from any Video Entry subject;

  3. the Video Entry is an original work of authorship and does not violate any third party’s proprietary or intellectual property rights.  If the content infringes on any intellectual property rights of a third party, the educator-student team shall be disqualified at the sole discretion of the Administrator and shall, at educator-student team’s sole expense, defend or settle such claims and indemnify, defend and hold harmless Administrator and the Releasees from all and against any suit, proceeding, claim, liability, loss, damage, costs or expenses arising from such infringement of any third party’s rights; 

  4. they acknowledge and agree that their Video Entry may be made available to other public online followers and, as such, subject to viewing, rating, and commenting by the public.  They acknowledge that comments or ratings that they disagree with or are unhappy about may be published or otherwise become associated with the Video Entry submitted. By submitting your Video Entry, Eligible Entrants waive any privacy expectations that they may have with respect to the Video Entry content; and

  5. publication of the Video Entry via various media, including posting on the Site, will not infringe on the rights of any third parties; and

  6. they agree that their Video Entry may be edited in the sole discretion of the Sponsor, including, for example, to make it shorter or to blur out any third-party brand logos that may inadvertently be visible.

  • Cannot feature more than three pieces of PII (Personally Identifiable Information) for students; Educators uploading the video must not disclose the full name and location of a student.
  • Must comply with the YouTube Privacy Policy, Terms of Service, and Community Guidelines available on the www.YouTube.com website.
  • Must comply with all Time To Milk It Official Rules contained herein.

 

Limit one (1) entry per eligible participant. At the conclusion of the Entry Period, contest judges will select one (1) Grand Prize winner to receive a smoothie bike personally delivered to their school by an NFL player.   • 40 points: Clear, compelling and creative expression of students showing how they “milk it” - We want to see the things that matter to you, things that milk helps you do – whether it’s art or science, sports, music or volunteering, etc.
  • 40 points: Clear, authentic message about the positive, nutritious aspects of milk
  • 20 points: Audio and visual quality (technical execution of video)

 

4. Prizes: Ten (10) First Prize winners will each receive a Fuel Up to Play 60-branded “Rock the Bike” Smoothie Bike. One (1) Grand Prize winner will receive a visit at their school from an NFL player who will personally deliver a Fuel Up to Play 60-branded Rock the Bike Smoothie Bike to the school. First Prize retail value: $200 per winner for a total First Prize value of $2000. Grand Prize retail value: $4,000. Total prize pool value: $6,000. Odds of winning depend on the number of valid entries received. Prizes are non-transferable. Federal, state, and local taxes, if any, are the sole responsibility of winners. 4. Prize: One (1) winner will receive two (2) tickets to a 2019 NFL regular season game in the market closest to the winner’s home. Ticket costs will be limited to no more than $400. Travel, lodging and meal costs are limited to no more than $1,500. Prize has an approximate retail value of $1,900. No cash or other substitution may be made, except by Sponsor in the event that a prize (or part thereof) cannot be awarded, in which case Sponsor will in its sole discretion award a prize (or part thereof) of equal or greater value.
